PARTICIPANT WAIVER & PARENT/GUARDIAN AGREEMENT

1. PARTICIPANT INFORMATION
Parent / natural or legal guardian name, phone number and email address.
Each participating child's full name and date of birth.
NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN
READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF LITTLE HAINES CITY-KICK & PLAY LLC USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM LITTLE HAINES CITY-KICK & PLAY LLC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND LITTLE HAINES CITY-KICK & PLAY LLC HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.
2. ASSUMPTION OF INHERENT RISK
Play at Little Haines City is active. Children run, climb, jump, slide, crawl, use role-play equipment and play alongside other children. Injuries such as bumps, scrapes, sprains, fractures and more serious harm can happen even when everyone uses reasonable care, because some risks are a natural part of active play. I understand these risks and choose to let my child take part.
3. RELEASE OF INHERENT-RISK CLAIMS
As the natural or legal guardian of each child listed above, and to the fullest extent allowed by Florida law (including § 744.301, Florida Statutes), I release LITTLE HAINES CITY-KICK & PLAY LLC and its owners, staff and agents from claims for injury, death or property damage that result from the inherent risks of these activities. This release does not waive any right that cannot legally be waived. Florida law governs this agreement.
4. RULES & MEDICAL
We agree to follow posted rules and staff instructions, and I understand grip socks are required in the play areas. Little Haines City is not a drop-off childcare: a responsible adult must supervise my child at all times. I confirm my child has no known condition that makes participation unsafe, and I authorize staff to give basic first aid and call emergency medical services if needed.
